Charge Entry Services

Every claim starts with a charge. When a visit is never charged, a code is mis-keyed or a modifier is missing, the error travels through the whole revenue cycle and shows up later as a rejection, a denial or lost revenue. Charge entry is simple work that is easy to get wrong when staff are busy.

Our charge entry team enters charges accurately and checks them against your documentation before they become claims. We also look for visits that were never charged at all. Charge entry feeds straight into our claim submission work, so what is entered is what gets billed. Our work runs at a 99.9% accuracy rate.

How our charge entry process works

  1. Receive charges. We pick up charges from your EHR or practice management system, or the format your team uses.
  2. Reconcile against the schedule. Visits are matched to charges so any visit with no charge is found and followed up.
  3. Enter accurately. Patient, provider, service, code, modifier and diagnosis details are keyed or validated against the source.
  4. Check against documentation. Codes and modifiers are reviewed against the note, order or encounter record, and gaps are sent back to your team.
  5. Release to claims. Reviewed charges move to claim submission, where they are scrubbed and sent to the payer.
  6. Report and improve. We track holds and recurring errors, and feed them back so the same mistake does not repeat.

Frequently asked questions

What is charge entry in medical billing?

Charge entry is recording the services provided at a visit, with codes, modifiers, diagnoses, provider and patient details, so a claim can be created and sent to the payer.

What is the difference between charge entry and charge capture?

Charge capture is identifying everything billable at the point of care. Charge entry is recording those charges in the billing system. We do the entry and also check that nothing billable was missed.

How do you make sure charges are coded correctly?

We review codes and modifiers against your documentation and send questions back to your team when the record does not support a charge. Complex coding questions go to our coding team.

Do you catch visits that were never billed?

Yes. We reconcile charges against the schedule so visits with no charge are identified and followed up.

Can you work inside our EHR or billing software?

Yes. We work in your existing systems and follow your workflow, so your team does not manage a second process.

Is charge entry separate from claim submission?

They are connected steps. Charge entry creates a correct charge. Claim submission scrubs and sends the claim and works rejections. We can do both, or just one, depending on your setup.

How do you protect patient information?

We work under HIPAA with MFA/VPN access, encryption, background checks and NDAs, and locked devices. Please don't send patient details through our website forms.
